In the Fall of 2020, the Disability and Aging Justice Clinic at the CUNY School of Law created an app to help people with disabilities who need to make healthcare decisions (including people with I/DD, age-related dementia and mental health challenges). The app provides information to decision-makers, healthcare providers, family members, and advocates so they can figure out how to make those decisions, or have their decisions accepted, without the need to appoint a guardian.

App for Healthcare Decisions in New York
Based on your answers, the app will give information and options for the healthcare decisions that need to be made, which will depend on whether the person in need of treatment has a developmental or intellectual disability, age-related dementia, or mental health challenges.
The app also gives definitions to help clarify important terms. In some question prompts, links connect you to helpful resources with more information.
